What does MGO 400+ mean in manuka honey?

MGO 400+ means the honey contains at least 400 mg/kg of methylglyoxal, the compound primarily responsible for manuka's non-peroxide antibacterial activity. MGO is a direct measurement of that compound. Higher numbers indicate greater concentration — MGO 400+ is considered a high therapeutic grade, well above entry-level manuka sold as table honey, which typically rates MGO 30 to 100.

What the numbers measure

Manuka honey grading exists because manuka has an antibacterial mechanism that other honeys lack, and the strength of it varies enormously between batches.

All honey has some antibacterial activity from hydrogen peroxide produced by an enzyme the bee adds. That activity is fragile — heat, light and the enzyme catalase in body tissue all destroy it.

Manuka has a second, non-peroxide activity that is stable. It comes from methylglyoxal, which forms from dihydroxyacetone present in manuka flower nectar, converting gradually as the honey matures.

MGO is a direct measurement: milligrams of methylglyoxal per kilogram of honey. MGO 400+ means at least 400 mg/kg.

What manuka does not do

It does not reach the lower airways or alveoli. A spray coats the oral cavity, throat and to some extent the upper pharynx. It does not travel down the bronchial tree, and any claim that it does is not physiologically plausible.

It is also not a treatment for respiratory infection, and it should not delay appropriate medical care.

One practical caution: honey is sugar. In a spray the total quantity is small, but anyone managing blood glucose closely should be aware of it. And honey should never be given to infants under twelve months due to botulism risk — irrelevant to an adult product but worth stating.
