Oxygen Saturation: What the Number on a Pulse Oximeter Means
A cheap fingertip device measuring something genuinely useful, with limitations that matter more than most users realise.
What is a normal oxygen saturation level?
Normal resting oxygen saturation (SpO2) in healthy adults is typically 95 to 100 percent. Readings of 91 to 94 percent may warrant medical discussion, and below 90 percent generally requires prompt evaluation. However, people with chronic respiratory conditions may have a lower normal baseline set by their physician. Pulse oximeters are less accurate at lower saturations and can be affected by poor perfusion, nail polish and skin pigmentation.
How the device works
A pulse oximeter shines two wavelengths of light — red and infrared — through your finger and measures how much of each is absorbed on the other side.
Oxygenated and deoxygenated haemoglobin absorb these wavelengths differently. From the ratio, the device calculates the percentage of haemoglobin binding sites carrying oxygen. It isolates the pulsatile component so it measures arterial rather than venous blood.
It is a genuinely clever piece of engineering that turned a laboratory measurement into a fingertip one.

Important exception: people with chronic respiratory conditions may have a lower baseline that their physician considers acceptable for them, sometimes in the high 80s. If that applies to you, your doctor's target is the number that matters, not the general range.

The limitations, which are substantial
Accuracy degrades at lower saturations. Oximeters are calibrated primarily against higher readings and become less reliable below about 90 percent — unfortunately the range where accuracy matters most.
Skin pigmentation affects accuracy. This has been documented in significant studies: pulse oximeters can overestimate saturation in people with darker skin, meaning genuine hypoxaemia may be missed. This is a recognised limitation of the technology, not a minor caveat.
Poor perfusion breaks it. Cold hands, Raynaud's, low blood pressure and vasoconstriction all reduce the pulsatile signal the device depends on. Warm your hands before measuring.
Nail polish and artificial nails block the light path. Dark colours are worst.
Motion produces artefact. Stay still.
Carbon monoxide poisoning gives falsely reassuring readings, because carboxyhaemoglobin absorbs light similarly to oxyhaemoglobin. In suspected CO exposure, an oximeter is misleading.
Measuring properly
- Warm your hands if they are cold.
- Remove nail polish from the finger you use.
- Sit still and rest for five minutes before measuring.
- Support your hand at heart level.
- Wait for the reading to stabilise — usually thirty to sixty seconds, not the first number shown.
- Take it at the same time of day if you are tracking trends.
- Record it alongside how you felt. Trends and context beat isolated numbers.
What a change in saturation actually indicates
Saturation reflects how well oxygen is crossing from air into blood. Reduced saturation points to a problem with gas exchange, ventilation, or the matching of the two.
It does not tell you about oxygen delivery, which also depends on haemoglobin concentration and cardiac output. Someone with anaemia can have a perfectly normal saturation while delivering substantially less oxygen to tissue, because saturation is a percentage of available binding sites rather than a measure of total oxygen carried.
This is why saturation is one useful number rather than a complete picture, and why breathlessness with a normal reading still deserves investigation.
