Steam Inhalation: What It Does, What It Does Not, and the Burn Risk
Genuinely comforting, frequently oversold, and responsible for a meaningful number of emergency department burns every year.
Does steam inhalation help a cough or cold?
Steam inhalation provides temporary symptomatic relief by humidifying airways, loosening secretions and soothing irritated mucosa. Trial evidence does not support it shortening illness duration or altering disease course, and results for the common cold have been mixed. Scald injuries from steam inhalation are common, particularly in children, so bowl-of-boiling-water methods carry real risk.
What it plausibly does
Warm humidified air reaching the upper airway has a few reasonable effects.
It adds moisture to mucosal surfaces, which is directly useful when dry air has thickened secretions. It can loosen crusted secretions in the nose. And warmth on inflamed mucosa is subjectively soothing in a way that is real even if it is not disease-modifying.
For symptomatic comfort during a cold or in a dry environment, these are not nothing. People are not imagining the relief.
What it does not do
Here is where the honest assessment matters.
Trial evidence does not support steam inhalation shortening the duration of a common cold or altering its course. Systematic reviews of heated humidified air for the common cold have produced mixed and generally unimpressive results on clinically meaningful outcomes.
It does not reach the lower airways or alveoli in any meaningful way. Steam condenses in the upper airway; it does not travel down the bronchial tree. Claims about steam clearing the lungs or detoxifying them are not physiologically plausible.
It does not kill viruses in the respiratory tract. The temperature required would injure tissue long before it affected viral viability.
The burn risk, which is not theoretical
Steam inhalation using a bowl of boiling water is a recognised cause of scald injury, and emergency departments see these regularly. The mechanism is usually a knocked bowl, and the injuries are to the face, chest and hands.
Children are at particular risk, both from spills and because they are less able to move away from steam that becomes too hot. Steam inhalation over a bowl should not be used with young children at all.
There have also been documented cases of significant airway injury from inhaling steam that was too hot, which is a more serious mechanism than an external scald.
Comfort measures plus actual mucolytic support

Safer approaches with most of the benefit
- A warm shower. Sit in the bathroom with the shower running warm and the door closed. Achieves comparable humidification with no scald risk. This is our default recommendation.
- A cool mist humidifier in the bedroom overnight. Steady humidification without heat, and better matched to the actual need. Clean it thoroughly and often.
- A bowl of hot — not boiling — water, placed on a stable surface, at a distance where it feels comfortable rather than intense. Never with children present.
- Saline nasal irrigation, which has better evidence than steam for nasal symptoms and no burn risk.
On added essential oils
Eucalyptus, menthol and similar additions produce a strong sensation of clearer breathing. That sensation is largely a cooling receptor effect — menthol activates TRPM8 receptors, producing a perception of airflow that does not correspond to an actual change in airway calibre.
Which is worth knowing: it feels like your airway opened, and measurably it did not. That does not make it useless as symptomatic comfort, but it does mean the sensation is not evidence of a mechanical effect.
Cautions: some essential oils are airway irritants, particularly at concentration and particularly in asthma. Camphor and eucalyptus products are not appropriate for young children, and camphor toxicity in children from topical and inhaled products is documented.
A reasonable position
Use warm humidified air for symptomatic comfort if you find it helpful, preferably via a shower or a humidifier rather than a bowl of boiling water. Expect comfort, not cure.
And do not let it substitute for assessment of a cough lasting more than eight weeks, breathlessness, or fever with rapid deterioration.
